Beech Grove Medical Practice

If you would like to register with the practice, please bring your medical card with you, but do not worry if you can't find it as you will be able to fill in a form at reception. The receptionists will be able to help you with any questions you may have.

All patients have a right to express a preference for a 'usual' doctor and may do so when registering.

All newly registered patients over the age of 16 will be asked to complete a questionnaire and have a consultation with the practice nurse before their registration is completed. Medical treatment is available from the date of registration.
